2021/4/13· The bile contained a large quantity of sludge. Infrared absorption spectrometry revealed that the sludge consisted of calcium bilirubinate (Fig. 1f). The amylase level in the bile was 5 U/L, and the lipase level was 66 U/L (24 and 8 U/L in the serum, respectively).
Calcium bilirubinate granules were observed in high (4'' counts) concentrations in eight and in low concentrations in the remaining 32 stone samples (1'' in 30, 3'' in two). Calcium carbonate microspheroliths were observed in 14 (35%) of 40 samples.Thmy gallstones showed high (43 cholesterol crystal counts with absent or low (13 counts of calcium bilirubinate granules and were classified as
It contains tiny sediments of calcium bilirubinate granules and cholesterol crystals. Mucus is also present and this is an important scaffolding for cholesterol stones. It should not be seen as a problem unless it is present for a prolonged period of time.
2021/5/26· Precipitation of cholesterol monohydrate crystals and calcium bilirubinate granules in the sites of the excessive viscosity of polymerized glycoprotein biliary mucin causes the formation of biliary sludge, the increase in its echogenicity and its revelation during
Calcium bilirubinate granules were present in gall bladder bile of 29 patients. They were present alone in all four patients with pigment stones (sensitivity of100%andspecificityof100%forpredictionof pigment gall stones); and in association with cholesterol crystals in
Microlithiasis can be defined as a suspension of precipitates of cholesterol monohydrate crystals or calcium bilirubinate granules in bile.1 Its presence can be suggested by transabdominal ultrasonography as hyperechoic non-shadowing mobile images, the so2 3
The small irregular aggregates are calcium bilirubinate granules. B) Microscopy of bile of a microlithiasis patient containing typical cholesterol monohydrate crystals but also many intermediate non-plate like (presumably anhydrous) crystals such as needles, arcs, tubules and spirals (courtesy of Prof. Dr. Piero Portincasa).
The authors consider that the observation of LLE in the gallbladder bile is an important index of the presence of cholesterol crystals and/or calcium bilirubinate granules. These LLE can assume polymorphic aspects, described by the authors, which may simulate other pathologies and lead to incorrect echographic diagnosis.

2018/4/1· Biliary sludge is a suspension of cholesterol monohydrate crystals and calcium bilirubinate granules in a matrix of mucous gel and bile. It is essentially diagnosed by ultrasonography, and it may play a pathogenic role in the etiology of biliary colic pain and acute pancreatitis.[ 3 ]
Brown stones are formed of calcium bilirubinate and are usually associated with biliary infection. They occur in both the gallbladder and the bile ducts. Black pigment stones are extremely hard bilirubin polymers and are found mainly in the gallbladder. Biliary sludge is a necessary precedent of gallstones.
None of the 16 patients free of stones had cholesterol monohydrate crystals or calcium bilirubinate granules in bile. Among the 17 patients with proven cholelithiasis, 13 with cholesterol stones had cholesterol monohydrate crystals in their bile, but only 2 of the 4 patients with pigment stones had calcium bilirubinate granules.

Calcium bilirubinate granules are almost invariably present, with bilirubin usually found in its unconjugated, least soluble form. The source of the unconjugated bilirubin is controversial. Bilirubin is excreted by the liver mainly in its diglucuronide form, but with small amounts of the monoglucuronide and unconjugated forms.
Calcium bilirubinate granules were identified on the basis of reddish-brown color (Fig. 2 B). Samples were considered positive for cholesterol or calcium bilirubinate granules if there were five
of granules of calcium bilirubinate. The sediment of the specimen from the gall bladder containing stones also contained granules of calcium bili rubinate. Fifteen dilutions of each bile specimen were prepared with 0.9% sodium chloride solu tion. The first
Microscopic examination of bile showed abundant precipitation of calcium bilirubinate granules in all samples. Thus, bile duct bile infection with beta-glucuronidase active bacteria (e.g. E. coli, C. perfringens) appears to be a key factor in PBDS pathogenesis, having a precursor role, rather than being a …
